We have received approval to be the first outpatient facility to start performing COVID-19 PCR testing at our facility with results in 45 minutes.
Kindly note that we are a doctor’s office, not a standalone testing site. We require a short doctors visit with your Covid-19 test. Due to the extremely high demand of testing requests, we cannot guarantee an immediate appointment. We will do everything possible to accommodate your requests.
Please email this completed 2-page registration form, along with a copy of the front a current ID and of an insurance card, and a contact phone number.
If all the required documentation is not submitted with your request, it will be rejected, and duplicate requests will be deleted.
We are also drawing for COVID antibodies. Please follow the same steps listed above to get on the schedule for this bloodwork. Having had a positive test result for Covid-19 is not a prerequisite for the draw. Please add any Covid-19 testing dates and results along with the registration forms and other required information.
